XMUT REPLACEMENT PROCEDURE OF A MISSING GRADUATION OR DEGREE DIPLOMA

Ministry of Education stipulates no replacement is issued when the original graduation or degree diploma is missing, but a letter of accreditation (with a similar content) can be issued. The application procedure of the letter of accreditation goes as follows:
1. To announce a notice of the lost diploma in a local newspaper.
2. Providing a copy of the newspaper, ID card and its photocopy, two 2-inch personal photos, replacement application form, and a photocopy of the original graduation documents issued by XMUT Archives.
3. All the materials should be provided by the diploma owner personally in case the photos need to be checked; downloading and filling the attached the replacement application form of a missing graduation or degree diploma.
4. The replacement accreditation letter can be received by appointed persons by post.
